PASSPORT  RENEWAL DS-82

You should use this form if:

1. You already have a passport that is not damaged; and
2. Received it within the past 15 years; and
3. Were at least age 16 when it was issued; and
4. Still have the same name as in passport (or you can legally document your name change).

To proceed with renewing your passport through IRTC, please send the following documents to IRTC:

  1.   Download IRTC Order Form

  2.  Complete online Application Form DS-82       

  3.  Your most recent passport

  4.  Two (2) identical passport-type photographs

  5.  Payment of US$170.00   (US$110.00 for non rush passport processing) 

• Check made out to U.S. State Department

  5.  Proof of Departure

• Airline ticket or an original itinerary will be needed (contact WDC travel department)
