FIRST TIME US PASSPORT  FORM DS-11 

You must use this form if:

1. You have never had a U.S. passport or

2. You are under age 16 or

3. Your most recent passport was issued more than 15 years ago or

4. Your most recent passport was lost or stolen. (If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64 download and fill out.)

To proceed with obtaining a new passport through IRTC, please submit the following documents to an official Witnessing Clerk (at a US Post Office or the County Court) 

Find the nearest authorized passport acceptance facility    

  1.       Download IRTC Order Form

  2.       Complete online Application Form DS-11

• Complete both pages of the application and print on one-sided pages (do not sign the application). 

• Bring pages 3 and 4 of the instructions along with the application when you appear at the acceptance facility. 

• One copy must be presented to the Post Office/ Clerk of the County Court at the acceptance facility. This will be sealed in an official envelope. 

• The second copy must be attached outside the envelope with your IRTC order form when it is sent to our office. Make sure to include all pages that print. 

  3.   Proof of U.S. citizenship (one of the following documents)

• A certified United States birth certificate

• An expired United States passport

• Original naturalization papers

  4.  Proof of identification issued by the Federal or State government

• State issued driver's license bearing a photo

• Military I.D. bearing a photo

• Government I.D. bearing a photo

  5.  Two (2) identical passport-type photographs

• Photographs must be official passport-type only and approximately 2" x 2" with a clear background. Face forward only.

  6.  Payment of US$157.00

• The witness sealing your documents, at the authorized passport acceptance facility, may require a portion of the money paid directly to their office as a processing fee, therefore bring two (2) checks.

  7.  Letter of Authorization

• This letter will authorize IRTC to submit and obtain the new passport on your behalf.    

  8.  Proof of Departure

• Airline ticket or an original itinerary will be needed (contact WDC travel department) 

You will sign the passport application form in the presence of the representative who will witness your signature and will place in an envelope the following items from the above list:

• Proof of citizenship

• Two (2) passport-type photographs

• Passport Application Form (Form DS-11)

• Payment

• Letter of Authorization

• Proof of Departure

The envelope will then be sealed by the authorized representative and given to you. Please send this envelope as well as the IRTC order form and the second copy of the passport application to IRTC.
