To obtain additional passport pages, please submit the following to IRTC:  

  1.  Download IRTC Order Form

  2.  Download Application Form DS-4085

  3.  Current passport in your possession (Passport must be signed.) 

  4.  Proof of Departure 

• Airline ticket or an original itinerary will be needed. (contact WDC Travel department)

  5.  Payment of US$ 142.00

• Check made out to US State Department
